Indications for application:

Cialis pills are used for treatment of patients with erectile dysfunction.  

Counter indications:

• in case of evidenced hyper sensibility to tadalaphyl, or any other ingredients, which are components of the medication;

• for patients, who are taking any medications, which contain organic nitrates;

• patients below the age 18 years old.  

Use in pregnancy and lactation:

Cialis is not made for treatment of women or children.  

Dosage regimen and mode of use:

Tablets of Cialis should be taken orally. 

Use in men of middle age

The recommended dose for treatment with Cialis is 20mg. The medication can be taken regardless of meals before expected sexual intercourse. The medication has to be taken at least 15 minutes before engaging in sexual activity.

After intake of Cialis patients are able to have successful sexual intercourse during the next 36 hours. By trying to have sexual intercourse your will be able to determine your individual time of reaction and time of best response.

It is recommended to take the medication not more often than once in 24 hours.  

Application of the medication for treatment of elderly patients and patients with impairment of liver or kidney functioning

There is no need to correct dose for elderly patients. Senior patients can follow the directions for use, which are applied for middle-aged men. Patients, who suffer from impaired functioning of kidney (with creatinine clearance of 30ml per minute) and those with problems with liver functioning don�t need any special dosage regimen (make notice of Special warnings and precautions section).  
 
 

Side effects:

The most often happening undesired side effects include dyspepsia and headache (7 и 11% of case respectively). In most of patients side effects, caused by the use of Cialis in patients were moderate and not significant in terms of expressiveness, they were going away after continuous use of medication, as they were transitory.

Other common side effects from use of Cialis include myalgia, backache, flashes of blood to face and stuffed nose.

In very rare cases patients suffered from pain in eyes, hydrops of the eyelids, faintness and hyperemia of the conjunctiva.  

Overdosing:

In case if Cialis is used in single-shot dose of 500mg in 24 hours for treatment of healthy patients and in case of repeated administration of the medication in the dose of 100mg in a day for patients with erectile dysfunction, the side effects were the same as when using regular dose for treatment. If there are cases of getting overdosed, it is recommended to treat the symptoms.  

Special notices:

For patients, who have diseases of cardio-vascular system, sexual activity can be a potential risk, for this reason treatment of erectile dysfunction, using Cialis is not recommended for men, who have any kind of heart problems, during which it is not recommended to be sexually active.

Patients with problems with cardio-vascular system should be aware that Cialis may cause aggravation of these diseases:

• stenocardia, caused by sexual intercourses or unstable stenocardia;

• myocardial infarction which happened within the last 90 days;

• uncontrollable impairments of cordial rhythm, uncontrollable arterial hypertension, hypotension (with AP

• class 2 and higher of cardiac insufficiency in terms of NYHA, which has developed within the last 6 months;

• insult which happened within the last 6 months. 

It is recommended to take the medication carefully for treatment of patients, who are predisposed to priapism (such as patients with multiple myeloma, crescent-cell anemia or leukemia), patients with anatomic deformation of the penis (for example, cavernous fibrosis, with angulation, or Peironi disease). Late treatment of priapism can cause development of liaisons in the tissues of penis and can cause long-term loss of potency.  

The safety and efficiency of the medication in combined use with other kinds of medications, applied for treatment of erectile dysfunction has not been determined. Combining such medication is not recommended, as the effect is not known.  

Influence of ability to guide automobile or use machinery:

No special precautions are necessary for driving automobiles.  

Pharmacologic properties:

Cialis is an effective, inversible selective PDE% inhibitor (specific phosphodiesterasa of the 5th type) cyclic guanosine monophosphate (cGMP). When sexual stimulation causes local liberation of nitrogene oxide and inhibition of PDE5 inhibitors, caused by tadalaphyl may cause increased level of cGMP in the penis and its cavernous body. As a result of such activity the medication causes relaxation of arteries and smooth muscles, and the reflux of blood in tissues of the penis. This gives positive effect for erection. Tadalaphyl does not have the effect without having proper sexual stimulation.  
 

Cialis has the ability to improve erection and gives possibility of having effective sexual intercourse. The medication has long-lasting effect for 36 hours. The effect from intake of Cialis appears already after 15 minutes of intake of the medication, but only if there is sexual stimulation.  

Tadalaphyl does not accurately cause changes in the frequency of heartbeats.  

Tadalaphyl does not have the ability to change the discrimination of blue and green colors. It also does not have any effect on vision, intraocular pressure, electroretinogram, and dimension of the pupil.  

When effect of Cialis was studied on men, the medication has should that it does not affect the quantity or the concentration of sperm, its mobility or morphology of spermatozoids.  

Pharmacokinetic properties

Tadalaphyl is rapidly absorbed after oral intake. The maximum mean concentration of the medication in plasma of blood is reached after 2 hours of oral intake.

The rate and speed of absorption of Tadalaphyl does not depend on intake of food and for this reason the medication can be taken with or without meals. It does not matter, if the medication is taken in the morning or in the evening, as it does not affect the absorption of the medication.

Metabolism

Metabolism of Tadalaphyl is done with participation of isoferment (CYP)3A4 cytochrome Р450. The main metabolite for circulation is methylcatecholglucuronid. The metabolite is less active than tadalaphyl at least 13000 times in terms of PDE5. For this reason, in case of such intake, this metabolite is not likely to be shown in clinical significance.  

Elder people

Healthy people of elderly age (65 years or older) have lower oral clearance of the tadalaphyl medication, which means that they need lower doses for treatment. In general elderly patients should take dose of 25% lower than for treatment of middle-aged men. There is no special precations necessary for treatment of elderly patients.  
 

Impairment of the liver function

There is no need to specifically correct the dose of Cialis for treatment of patients with moderate or weak problems with liver or liver impairment.  

Patients with diabetes

There is no need to specifically correct the dose for treatment of patients, who have any kinds of problems with diabetes, as this does not affect the concentration time of tadalapnyl significantly.  

The medication expires in two years and should not be used after the expiration date, which should be indicated on the package.
