3. Indications for Use.
Climacteric osteoporosis, osteodystrophy of kidney genesis, idiopatic and post-surgical hypoparathyreosis, pseudo-deficient vitamin-D-dependent rachitis, pseudohypoparathyreosis, native vitamin-D-resistent rachitis (phosphate-diabetes).
4. Action
The medication is of D-vitamin-form. It has the effect of enhancing the calcium absorption in the intestinal tract, as well as it stimulated the osteoblasts and regulates mineralization of bones.
5. Posology and Administration
The medication is for oral intake. The average daily dose should be defiled according to the level of calciemia. The dose for start of treatment is 0.25mkg and then can be increased to reach the level f average dose of 0.5-1mkg per day or higher. For treatment of climacteric osteoporosis it is recommended to take medication 2 times a day with dose of 0,25 mkg. For treatment of kidney osteodystrophy the daily dose for start of treatment is 0,25 mkg. Dose of 0.25mkg is enough for treatment of patients with a normal level of calcium serum or for patient with imperceptible decreased level. For treatment of rachitis and hypoparathyreosis the starting dose should be 0,25 mkg daily which should be taken in the morning. In treatment of phosphate-diabetes use a supplementary indication of phosphorus-containing drugs. For treatment of children it is recommended to use 0,01–0,1 mkg per 1kg.
6. Counterindications
Hypersensitivity, hypercalciemia, hyperphosphatemia, kidney osteodystrophy accompanied by hypervitaminosis D.
7. Should Be Taken with Precautions, if…
The content of calcium and creatine should be defined in 4 weeks, as well as every 3 and 6th months of the treatment start. In future treatment check up should be done once in half a year. Administration of daily dose of 5mkg should be done with additional care (determination of calcium level, clinical trial, level of phosphoros and activity of alkaline phosphatase in blood has to be performed twice per week).
It is necessary to be especially cautious in case of active form of pulmonary tuberculosis, atherosclerosis, hyperphosphatemias, chronic disease of heart, phosphate nephrourolithiasis, chronic diseases of kidney, pregnancy or period of lactation, sarcoidosis or other granulomatoses, childhood age and for treatment of elderly patients (as it can cause developmend of atherosclerosis).
8. Interface with Other Medications
The medication can be used with some types of drugs, but for some other medications it has the ability to enhance toxicity. In order to eliminate possibility of intoxication, inform your doctor about all medications, which you are currently taking or have taken in the recent past, or those, which you are planning to take.
9. Overdosing
Symptoms of overdosing include chronic intoxication: dysesthesia, dystrophy (loss of weight, weakness), eventual fever with thirst, water deprivation, polyuria, apathy, infections in urinary tracts, retardation of growth. Focal calcification of cardiac muscles, cortical layer of kidneys, duodenal and lungs are after-effects of hypercalciemia.
If eventual overdosage happens, it should be treated by means of immediate lavement of stomach or use of a vomitive in order to avoid further absorption. If you suspect that you got overdosed, inform your doctor as soon as possible.
10. Side Effects
Anorexia, hypercalciemia, headache, gastralgia, vomiting, intestinal colic, increment of liver transaminases activity, calcinosis of soft tissues, constipations, dysesthesia, dystrophy, fever, polyuria, thirst, water deprivation, apathy, dizziness, retardation of growth, urinary tract infections.
11. Pregnancy and Lactation
It is allowed to use Rocaltrol for treatment of pregnant women only in case if the expected favor exceeds the possibility of harming the development of fetus.
Rocaltrol is excreted into human milk. It is only allowed to use Rocaltrol for treatment of breast-feeding women, when mother is aware of possibility of developing hypercalciemia and about side effects, which can develop in an infant, and under strict medical control and monitoring of concentration of cerum and calcium in the baby and the mother.
12. Driving
Taking into account the effect of pharmacodynamics and the side effects, mentioned above, the medication is not known to cause side effects, which can lead to drowsiness or dizziness and can affect ability to drive and operate machinery.
