3. Indications for Use.

Used for treatment of arterial hypertension (as a part of combined treatment of as mono-therapy)
Chronic failure of heart (as a part of combined treatment) 
Ischemic disease of heart: stable angina. For treatment of patients, who have ischemic disease of heart Carvedilol can be used for achieving anti-anginal and anti-ischemic effect, while for patients who have arterial hypertension the medication can be used to lower the tension. The medication has the ability to lower the preload (pressure on pulmonary artery wedge and pressure on pulmonary capillaries) and afterload (overall resistance of peripheral vascular system). Carvedilol is an effective medication, which is well tolerated by people, including those, who have chronic failure of heart of different levels of severity. For treatment of such patients it has the ability to reduce mortality.

4. Posology and Administration

The medication should be taken orally with a decent amount of water or other liquid. In case of treatment of arterial hypertension it should be taken one time a day for a period of two days in dose of 12.5mg, and then it is necessary to take 25mg daily with possible further gradual increase of dose at minimum two week interval. For treatment of stenocardia the medication should be taken one time a day for a period of 2 days in the dose of 12.5mg, with later increase of dose to 25mg two times a day for further treatment. The dose can be then gradually increased at an interval of two weeks at least until reaching the highest allowed dose of 100mg, which should be split in two intakes. For treatment of chronic failure of heart (in matched combined therapy under pressure) the medication should be used at starting dose of 3.125mg, which should be taken twice a day for a period of 2 weeks, and in case of good tolerance the dose of medication can be increased up to the level of 6.25mg, which can be taken 2 times a day. The dose can be gradually increased up to 12.5-25mg, which can be taken twice a day (for people with body weight below 85kg the highest daily dose is 25mg, taken twice a day, and for people with body mass above 85kg the allowed daily dose is 50mg, taken twice a day). If the treatment is for some reason interrupted after more than two weeks, it should be resumed with the dose of 3.125, taken two times a day, and with further increase of dose.

5. Counterindications

Second and third degree of AV block, hypersensitivity, failure of liver, evidenced bradycardia, cardiogenic shock, SSS, bronchial asthma, period of lactation, low arterial tension, young age below 18 (efficiency and safety of use of the medication have not been determined).

6. Should Be Taken with Precautions, if…

The medication should be taken with special caution for treatment of patients with ischemic disease of heart, who are using contact lenses, due to the fact the treatment with Carvedilol can cause lowering of delacrimation (this group of patients needs special attention). It is recommended to withdraw the medication gradually, as there is a chance of emergence of withdrawal syndrome. It should be withdrawn for a period of several days, especially when treating patients with ischemic disease of heart. It is not recommended to drink alcoholic beverages during treatment.

7. Interface with Other Medications

It is not allowed to use Carvedilol in combination with any other medications without order of a doctor.

8. Overdosing

In case of getting overdosed: bradycardia (lower than 50beats per minute), evidenced drop of arterial tension, impairment of respiration (including bronchospasm), cardiogenic shock, chronic failure of heart, heart arrest.

9. Side Effects

At the start of the treatment (effect of first dose) and in case of increasing the dose � possibility of having episodes of excessive drop of arterial tension. As a usual such side effects go away themselves. When the dose is corrected, such conditions usually go away. In case of continuous use of Carvedilol for treatment, there is a chance of developing fatigability, weakness, headache, rise of intestinal motility and hypersthenia, bradycardia; in less often cases there are possibilities of having conditions of peripheral blood flow disorder, attacks of stenocardia, apisodes of intermittent lameness, AV block, as well as allergic exanthema, hives, itching, reactions similar to lichen-planus, aggravation of previously existing process of psoriasis or emergence of psoriasic patches. In individual cases there were noticed conditions of psychic depression, sleep disorders, paresthesias, stuffiness of nose, enhancement of heart failure manifestation, as well as blood composition disorders.

10. Pregnancy and Lactation

Carvedilol was not properly studied to be used for treatment of pregnant women, for this reason it is only allowed to use the medication in such cases, when the expected positive outcome for mother exceeds the risk for fetus. It is not recommended to breast-feed during treatment with Carvedilol.

11. Driving

At the starting stage of treatment and when increasing the dose of Carvedilol used for treatment, it is better to keep away from vehicle driving, as well as from any kind of activities, which need elevated attention concentration and speed of psychic and motor reactions.
