Pharmacologic properties Pharmacodynamics Penile erection is a haemodynamic action, which is based on relaxation of carvenous bodies and smooth muscles in them, as well as arterioles, which are located in carvenous bodies. Oxide of nitrogen is evolved in the process of sexual stimulation and causes increasing f cyclic huanosine monophosphate. This process gives effect of relaxation of smooth muscles in bodies of carvenous, which increases the reflux of blood to the penis. The medication has the ability to block specific fifth type of phosphodiesterase (called PDE5 inhibitors), it takes part in local action of nitrogen endogen oxide in the bodies of cavernous in the process of sexual stimulation. Due to this effect Levitra medication has the ability to stimulate and intensify the sexual reaction to stimulation. Pharmacokinetics Levitra is easily absorbed after intake and the effect of the medication can be observed already after 15 minutes of intake (usually the desired effect is reached in 25 minutes after an intake) and the effect lasts for 4-5 hours after intake. Indications to application Should be used for treatment of erectile dysfunction (inability to receive and maintain erection, which is necessary for successful intercourse). Counter indications Hypersensitivity to any component of Levitra. Simultaneous intake of medications, containing nitrates and medications, which are donators of oxide of nitrogen. The medication is not recommended for combined treatment with protease HIV inhibitors, for example ritonavir or indinavir. Levitra should not be applied for treatment of women or children under the age of 12 years. It should be used with care in patients: with native OT interval prolongation; with anatomic deformation of the penis (cavernous fibrosis, contortion, Peironi disease); with ilnesses predisposing for priapism (multiple myeloma, crescent-cell anemia, leukemia); patients with severe problems with functioning of liver. Change of regimen of treatment and doses is not necessary for patients with moderate level of liver dysfunction. Patients with moderate level of liver problems should start treatment with 5mg taken once in 24 hours. Depending on effectiveness of treatment and individual reaction to the medication, it is allowed to increase the dose for treatment until reaching the level of 10mg; patients at terminal stage of kidney problems. No need to correct dosage for patients with moderate, minor and severe problems of functioning of kidney; Patients with arterial hypotension (in case id systolic pressure at rest is below 90mm of mercury column); patients, who recently suffered from myocardial infarction or insult or patients with instable stenocardia; patients with degenerative hereditary retinopathy, such as pigmented retinitis; patients who are predisposed to aggravation of bleedings and ulcer diseases; patients, who have idiopathic hypertrothic subaortal stenosis or aortal stenosis. Mode of application and doses The dose for start of treatment is 10 mg. The medication should be taken 25 minutes � 60 minutes before having sexual activity. The length of effect from Levira is 4-5 hours. The medication should not be taken more often than once in 24 hours. Depending on the effectiveness and tolerance of treatment by a patient, the dose can be either increased up to 20mg per day or lowered up to 5mg per day. The highest allowed daily dose is 20 mg. Recommended dose for intake by elderly patients (age above 65 years) is 5mg. Highest dose of 20mg should be used for treatment of patients with serious erectile dysfunction and patients who have prostatectomy or diabetes, and in case if dose of 10mg is insufficient. There is no affect of meals and alcoholic beverages of the work of Levitra. Levitra is only effective when there is enough sexual stimulation. Side effects Levitra is generally well-tolerated. And side effects, which emerge during treatment, are usually minor and moderately expressed. Side effects usually have transitive character. Most common side effects are (1—10%): redness of face, headache, dyspepsia, faintness, nausea, rhinitis. Not common and rare (0,1—1%): photosensitization, hydrops of the face, aches in the lumbus, changes in functional tests of liver, arterial hypertension, increased level of the GGTP, drowsiness, increased level of creatine kinase, dyspnoea, myalgia, disturbances of vision, priapism, runny eyes, anaphylactic reactions (for example such as laryngeal edema), arterial hypotension, cardiac angina, myocardial ischemia, syncopal conditions, postural hypotension, increase of muscle tonus, glaucoma, nose bleeding. Overdosing When Levitra is taken in single-shot doses above 80mg, there were no serious side effects developed. In the same conditions intake of 40mg of the medication twice a day, there were such side effects as marked witnesses pains of lumbus, which had no significant signs of toxic action on the nervous and muscular system. Interaction with other medications The dose of Levitra should not be more than 5mg, if the medication is used for combined treatment with ketoconazol, erythromycin, itraconasol. At the same time it is recommended not to exceed the 200mg doses of intraconasol and ketoconazol. It is not recommended to combine the medication with ritonavir and indinavir. Donators of oxide of nitrogen and nitrates: it is not recommended to intake LEvitra stimulataneously with nitrates as there is a big risk of developing arterial hypotension. Combined treatment with Levitra and alpha-adrenergic blockers may cause development of arterial hypotension. It is not recommended to take Levitra medication earlier than 6 hours after intake of alpha-adrenergic blockers and the dose of Levitra should not be higher than 5mg. In case if tamsulosine is used for treatment in combination with Levitra, the intakes can be combined.
