- Indications for Use.
Used for treatment of syndrome of Parkindonism, except for the one caused by neuroptics, Parkinson’s disease.
- Action
The medication help reduction of immobility, favors mental pertification, as well as lowers the inflammatory process in the cavity of moutn, inflammations, tremors of body, limbs, head, salivation and other signs of Parkinsonism.
- Posology and Administration
The dose for treatment with the medication should be defined individually, depending on the optimal concentration in the plasma of blood. The dose should also be chosen in order to avoid development of side effects.
The medication should be taken either with some amount of liquid or with meals. The tablets or capsules should be taken whole. The desired effect of treatment is usually acheved after one week of non-stop intake of the medication.
Patients, who have not previously been treated with Levodopa, the treatment should start with smaller dosage. Then the dose should be slowly increased once a day or once in two days until reaching the optimal dose (individual for every patient). As for Levodopa, the daily doses can vary from starting dose of 150-250mg per day, but it should not be higher than 2g of Levodopa per day. The usual dose for treatment with Carbidopa, which is needed to suppress the peripheral change of Levodopa is from 70 to 100mg. The highest allowed dose of Carbidopa is 200mg. Ir is recommended to split the daily dose into several intakes. The optimal effect is usually achieved after one week of treatment with intake of medication non-stop. For use of Levodopa, the dose should be from 750 to 1500mg per day. In case of having convulsions and closures of lids of eye it is necessary to reduce the dose.
When transferring from average use of Levodopa to a combined treatment of Levodopa with Carbidopa, it is recommended to stop administration of Levodopa at least 8 hours before starting combined administration, while when transferring from long-term use of Levodopa, minimal time is 12 hours.
A daily dose of the drug should be adjusted in such a manner that it could be roughly 25% from the previous daily dose of Levodopa.
When the daily dose of Levodopa medication is lower than 1500mg, then for majority of patients the initial dose of Carbodopa � Levodopa is 300-400mg of Levodopa per day. If further correction of regimen of dosage is necessary, the daily dose can be either decreased or increased by 125-250mg of Levodopa per day. The medication gives optimal performance from treatment after 7 days of non-stop intake. When patient develops convulsions of muscles and closure of lids of eyes, the dose should be reduced.
The supporting dose should be corrected, depending on clinical effect. If it is necessary to increase the dose of Carbidopa, they should be used at ratio of 4/1 with Levodopa / Carbidopa. If Levodopa dose needs to be increased, they should be used at ratio of 10/1 Levodopa / Carbidopa.
The dose for supporting treatment is 300-400mg of Levodopa per day for an average patient.
- Counterindications
- evidenced hypersensitivity to any components of the medication;
- severe psychoses, neuroses, depressions;
- closed angle glaucoma;
- cutaneous lesions with unspecified diagnosis;
- essential tremor, Huntington’s disease;
- melanoma (also assumption of it);
- simultaneous intake of monoamine oxidase inhibitors;
- pulmonary heart, heart failure;
- evidenced compromised functions of kidneys or liver;
- blood diseases, leucopenia;
- period of pregnancy and lactation;
- diseases of endocrine system;
- childhood (age under 12 year old, and the medication is not recommended for intake in patients under 18 years of age);
- secondary Parkinsonism, application-specific of neuroleptics (anti-psychotic medications).
Limitations of use: arterial hypertension, evidenced atherosclerosis, myocardial infarction with disturbances of rhythm, bronchial asthma, convulsive syndrome, lung diseases, erosive-ulcer of gastrointestinal tract.
- Should Be Taken with Precautions, if…
The medication should be carefully used with dose adjustment for treatment of elderly patients and people, who use other medicines.
The medication should be used with care for treatment of patients, who have severe conditions of lung disease, cardiovascular diseases, arterial hypertension, in manifested general atherosclerosis, bronchial asthma, manifested compromised functioning of kidneys and liver, lung diseases, endocrine diseases, convulsive syndrome, evidenced ulcer disease.
The medication should be taken carefully for treatment of patients, who just went through myocardial infarction and those, who still have remaining nodal, atrial, or ventricular arrhythmia. For patients in such condition it is necessary to regularly monitor performance of heart, particularly at the start of treatment, when dose is adjusted.
In case of long-term use of medication, it is necessary to have regular check up to blood formation, functioning of liver, kidneys and work of cardiovascular system.
Patients in psychotic conditions, as well as patients, with mental affections, need special care during treatment. It is necessary to closely monitor patients, who have mental disorders or diseases with an early depression detection and those with eventual suicidal inclinations.
Patients, who suffer from open-angle glaucoma should use the combination of Carbodopa and Levodopa cautiously, and with constant monitoring of intraocular tension and observation of the changed in process of treatment.
It is recommended to withdraw the medication gradually.

Interface with Other Medications
It is not allowed to take any other medications without order of a doctor, when being treated with Carbidopa and Levodopa medications.
- Overdosing
Symptoms of getting overdosed: nausea, convulsive tic, vomiting, convulsive closing of eyelids, diarrhea, loss of consciousness, frequent and irregular pulse, convulsions, hallucinations, agitation, coma. If a patient becomes unconscious, it is necessary to call ambulance immediately.

Side Effects
Side effects, which can appear during use of pregnancy and lactation or Levodopa are usually easy to avoid by reducing the dose, while some of them go away, as treatment continues. The reduction of dose is necessary in case of development of muscle convulsions, having sticky eyelids, different disorders of motion functions. If side effects become persistent, it is necessary to discontinue treatment.
Side effects on nervous system and mind: dizziness, headache, insomnia, unrest, drowsiness, nightmares, mania, anxiety disorders, euphoria, fainting fit, asthenia, lethargy, hallucinations, paranoid ideation, psychotic conditions, convulsive conditions, depression with or without the development of suicidal attempts, motion disorders.
Side effects of vision disorders: dilatation of pupils, blurring of vision.
Side effects on gastrointestinal tract: vomit, nausea, anorexia, dysphasia, hiccup, elevated salivation, burning sensation in the mouth, bitter taste, dryness of mouth, constipation, diarrhea, meteorism, pains in the pit of the stomach; bleedings from gastrointestinal tract, ulcers formation in gastrointestinal tract.
Side effects on cardiovascular system: rare – heartbeat, bradycardia, tachycardia, compositional disorder of blood.
Side effects on breathing system: huskiness, breathing rhythm disturbance.
Side effects on urogenital system: urinary incontinence, urination retention, haematuria, cases of long-lasting erection in men.
Dermatologic skin reactions: edemas, perspiration, rash, loss of hair.
Other side effects: hyperemia of face, hot flash, weight gain, elevation of libido, phlebitis. It is also possible to have discoloration of urine, when using the medication.

Pregnancy and Lactation
Combination of Carbidopa and Levodopa is only allowed for administration during pregnancy in case of vital necessity. If there is a need to administer medications in period of lactation, breast-feeding should be discontinued.
