Can be used for treatment of generalized or partial tonoclonic attacks as a part of combined therapy with other anti-convulsive medications or in monotherapy; in first diagnosed epilepsy (in children above the age of 2 years and adult patients), additional treatment for curing of Lennox-Gastaut syndrome fits (in children and adult patients).

5. Posology and Administration

Topiramate is a medication which has to be taken orally with or without meals. For treatment of adult patients in case of monotherapy, it should be used in dose of 25mg once a day in the evening before going to bed for start of treatment. And after one week of such treatment the dose can be slowly increased at 1-2 week interval until reaching the desired clinical effect. The recommended dose for treatment is 100mg per day, with the maximal daily dose of 500mg (some patients, suffering from refractory forms of epilepsy should use the daily dose of 1000mg). Children at age above 2 years old should take 0.5-1mg per one kilo fo weight per day during the first week of treatment, which should be taken before going to bed. The dose then can be increased by 0.5-1mg per kilo per day, which should be divided into several intakes. The dose can be increased at 1-2 week interval. The recommended dose for children is 3-6mg per 1 kilo per day (for treatment of children with recently diagnosed partial fits the dose can be up to 500mg per day).
When the medication is used as part of combined treatment with anti-convulsive medications is should be used in dose of 25-50mg for treatment of adult patients, which should be taken once a day before going to bed for a period of 1 week. In future treatment the dose can be increased by 25-50mg at a 1-2 week interval. Such increased dose should be divided into several intakes. The minimal dose, efficient for treatment is 200mg per day, with the average treatment dose of 200-400mg per day, and the highest allowed daily dose is 1600mg. For treatment of children above 2 years of age the medication should be taken in dose of 1-3mg per 1 kilo per day, which should be taken one time a day before bedtime for a period of 1 week. The dose can be then increased gradually once in 1-2 weeks by 1-3mg per 1 kilo. Such dose should be taken in 2 intakes. The daily dose recommended for treatment is 5-9mg per 1 kilo of body mass.

6. Counterindications

Topiramate is counter indicated in case of intolerance to any components of the medication, periods of breast-feeding and pregnancy, childhood age under 2 years old (as efficiency and safety of use of the medication have not been determined yet).

7. Should Be Taken with Precautions, if…

The medication should be used with caution for treatment of patients, who suffer from compromised functioning of liver. If patients develop significant weight loss during treatment with Topiramate, it is necessary to use high calorie diet during treatment and eat more food. Topiramate is not recommended for simultaneous intake with alcoholic drinks and medications, which suppress the work of central nervous system.

8. Interface with Other Medications

Topiramate should only be used in combination with other medications, if it was specified by a treating doctor, as such combinations can lead to adverse side effects.

Side effects on sense organs and nervous system: problems with balance in standing, increased fatigability, disorder of attention concentration and thought disorder, emotional instability, dizziness, consciousness fog, feeling of skin numbness, drowsiness, reduction of a superficial cutaneous touch; rare – amnesia, agitation, disorder of speech, depression, conjunctivitis, diplopia and other impairments of vision, perversion of taste, high speed unintentional oscillatory movements of eyes (can reach up to several hundreds per minute).
Side effects on organs of digestive tract: gingivitis, loss of appetite, nausea.
Other side effects: stomach pains, lowering of the body mass, cold fit, edemas, lowering of the number of leucocytes in blood, nose bleeding, panting, formation of stones in renal tubes or in renal pelvis, itching in the area of groin, presence of erythrocytes in urine, loss of hearing sensation, weakening of libido, painful menstruations, impotency.

11. Pregnancy and Lactation

Topiramate is not allowed for use during both period of pregnancy and breast-feeding.

Topiramate should be used with additional caution for treatment of patients, who are involved in activities, which can be potentially hazardous and those, which require speed of psychomotor reaction and alertness, as the medication has the ability to cause drowsiness.
